Register a domain first. Once registered you can point your mx records/dns records to Google’s dns server.

All you need is a Google account to use Google Apps.
First you need to buy a domain name and then goto where you can continue the signup process and register your domain with google apps. The registration will provide you with admin user name and password using which you can log into the control panel and can manage your email accounts.
In order to verify that the domain is yours google apps ask you to configure the site, one being uploading a html file to your website with code given by google apps.
